What is Social Prescribing?

At Almond Road Surgery we are fortunate to have Social Prescribers work with us as part of the A1 Primary Care Network (PCN). They work closely with our surgery, to give patients further, non clinical, support with a multitude of issues that they may have. The Social Prescribers are able to give advice, signpost or refer patients to a whole host of support for their individual needs.
